BENEFIT DESCRIPTION


Health Benefits Health benefits packages, including prescription coverage, for the employee and family through a preferred provider organization (nationwide and local networks).


Dental Benefits Dental benefits package for preventive/diagnostic, basic, and major services.


Vision Benefits Vision benefits package for examinations, contacts, and frames.


Flexible Spending
Accounts
Optional flexible spending account plan that allows employees to contribute pre-tax dollars to cover non-reimbursed medical and dependent care expenses.


Short Term Disability Insurance Company-paid short-term disability plan to insure income up to 66 2/3% of monthly earnings for prolonged injury, illness, or maternity (up to 90 days).


Optional Short Term Disability and Cancer Protection Plan Option to participate in a supplemental short-term disability and cancer protection plan. May be used in addition to company-sponsored short-term disability plan.


Long Term Disability Insurance Company-paid long-term disability plan to insure income up to 66 2/3% of monthly earnings for employees who become disabled for longer than 90 days.


Term Life Insurance Company-paid term life insurance:
  - First year: One times annual     salary
  - After first year: Two times     annual salary


Voluntary Term and Universal Life Insurance Option to purchase supplemental life insurance for employee, spouse, and/or dependent children.


Voluntary Accidental
Death & Dismemberment Insurance
Option to purchase supplemental accidental death and dismemberment insurance for employee, spouse, and/or dependent children.


401 (k) Plan Tax-deferred 401 (k) plan with matching company contributions. Employees may contribute 1% to 97% of salary, up to $15,000 annually. Over age 50 catch-up provision of $5,000.


Employee Assistance Program 24-hour access to confidential professional counseling services for personal, family, emotional, mental and behavioral issues.


Educational Assistance Program Tuition assistance for undergraduate and graduate level courses (after one year of employment).
  - Undergraduate: $1,500 per      year
  - Graduate: $3,500 per year


Section 529 College Savings Plan Option to participate in a Section 529 College Savings Plan for higher education expenses.


Accrued Annual Leave Based on years of service, 3 to 5 weeks of accrued leave may be taken for vacation, sick, or other personal reasons.


Paid Holidays Eight company-paid holidays (full-time employees). Four paid “floating” holidays per fiscal year.
